About:
The Contra Costa Regional Health Foundation (CCRHF) provides funding for CCH-related projects that are innovative, mission-aligned, support the Vital Conditions for Wellbeing, and have no other means of support. These grants are intended to support efforts that go beyond standard operations, where a clear need exists and a practical solution is ready to be implemented.

Before applying, proposals must be approved by executive staff and submitted with division manager sign-off.
Strong applications clearly define the problem, the population served, and the outcomes the project is expected to achieve. We look for projects with a clear lead, a realistic timeline, and a plan for implementation. Applicants should outline other funding sources they’ve explored, provide a detailed budget, and demonstrate why foundation support is needed. Projects are also expected to include a plan for sustainability and a clear approach to tracking progress, measuring success, and reporting outcomes.
